WAIVER AND CONSENT

I confirm that I own the Cat or I was given authority by the owner for taking ownership of the pet.
I assure All About Cats, LLC that this cat is currently in good health condition.
I confirm that my Cat has a complete and updated vaccinations.
I acknowledge that the Cat should have a safety collar.
I confirm that my Cat doesn't have any fleas or any communicable disease.
I release All About Cats, LLC, its employees, and owners from any and all liabilities but not limited to injury, sickness, damage, accident, or death while boarding or in the vicinity.
If ever that I wasn't able to pick up my pet on time, All About Cats, LLC will be calling the owner and get authorization for the Cat to stay with All About Cats, LLC at the owners expense to the credit card on file.
If my Cat needs medical attention, I authorize All About Cats, LLC to have it examined by an emergency veterinarian, at the owners expense.
If you will requiring additional days (depending on availability) during the holidays, please make sure to reserve at least week in advance. We cannot guarantee an extension of stay but will do our absolute best to accommodate.
